EXECUTIVE SUMMARY
BACKGROUND:
Attached for Council considerauon is Change Order No. 1 for the 2006 Pavement Management Project,
City Project #396 Please see the attached letter from Project Manager Kevin Kawlewski of WSB
Associates for a detailed explanation of the items relating to Change Order No. 1.
The above changes to the project result in a net increase of $18,491.31 for a revised contract amount of
$1,076,552 52
SUMMARY:
Staff recommends Council approval of Change Order No. 1 for the 2006 Pavement Management Project,
City Project #396.

Dear Mr. Brotzler:
Please find enclosed Change Order No. 1 (four copies) for the above referenced project. The
change order reflects a net increase to the contract amount of $18,491.31 for a revised contract
total of $1,076,552.52 If the attached meets with your approval, please sign and process.
The change order consists of the following:
Item 112 Maintenance of Sanitary Sewer Service: Inspection of a sanitary sewer service
found to be in an unanticipated location during construction to remove conflict
with hydrant installation.
Item 113 Repair Gate Valve: The Contractor was requested by the City maintenance staff
to excavate and remove dirt and debris that prevented operation of an existing
gate valve. This work was not included in the original project scope.

Repair Landscaping: This Item including restoration of miscellaneous landscaped
areas throughout the entire project impacted by new curb construction.
Relocate Sanitary Sewer Service: Relocation of the sanitary sewer service
conflict Identified in item 112.
Replace Curb Box: A damaged curb box was identified underneath a segment of
sidewalk to be reconstructed and was repaired at the direction of City
maintenance staff prior to reconstruction of the sidewalk.
Item 117 2" Caliper Tree: Two trees were installed in the pond area on upper 149 to
replace existing trees that were removed as a part of the project. Tree
replacement was not included as a part of the original contract, however these
trees were installed at the homeowners request.
Item 118 Mill and Overlay Bituminous Path: This work was requested by City staff to
repair failed sections of bituminous pathway along Chippendale Avenue and

Item 119 Lower Sanitary Manholes: These manholes were originally planned to be
reconstructed. It was identified during construction that a cost savings could be
realized on the project by adjusting the manholes rather than reconstructing them.
This item reflects payment for the adjustment. No payment was made for the
reconstruction of these manholes.
Item 120 Subgrade Excavation: The Contractor encountered small area, approximately 150
feet, of unsuitable materials under the existing pavement on the north end of the
project. This area was scheduled for rehabilitation, and so therefore no subgrade
excavation was included in the original contract.
Item 121 Remove and Replace Storm Sewer: Following excavation, the contractor
identified a conflict with an existing gas main that required the re- installation of a
section of storm sewer.
