Subject: Pandemic Flu Mini -Grant
The Dakota County Public Health Department and Dakota County Emergency
Management offered mum -grants to mumcipahties within Dakota County. Grants are
available in the amount of up to $12,000 The overall goals of the muu -grant initiative are
to:
1. Increase community awareness and preparedness around pandemic influenza and personal
and family preparedness.
2. Develop collaboration among municipalities and community partnering organizations such
as schools and community -based organizations.
3. Increase community readiness by developing and exercising Continuity of Service Plans.
A grant was submitted on behalf of the City requesting $12,000. The dollars requested m the
grant include:
$8000 for hiring of consultant
$3500 for copying, prmtmg, mailing of educational materials
$500 for meeting related expenses, e.g refreshments, room rental, publicity
The consultant recommended to be hired for this project is Merle Lohse Mr. Lohse retired
from the Apple Valley Pohce Department in May of 2005. For the last four (4) years of his
career, Merle was the Emergency Management Coordinator for Apple Valley and has
experience m wnting emergency operations plans and conducting table top exercises.
Mr.Lohse's services are will be utilized specifically for meeting three of the grant's
requirements
1. Develop a Continuity of Service annex to the Emergency Operations Plan.
2 Develop a Continuity of Service flow chart
3. Design, coordinate and conduct a multidisciplinary pandemic related tabletop exercise
and development of an After Action Report and Corrective Acton Report following the
exercise.
Mr. Lohse will assist in developmg educational materials but community meetings and other
educational efforts will be conducted by city staff. A contract for Mr. Lohse is expected to
be presented to Council at the January 16, 2007 Council Meeting.

Introduction:
Following the emergencies of September 11 and Katrina, government agencies and communities
have increased efforts to prepare for potential emergencies. The possibility of a pandemic influenza
has heightened the need to inform and prepare the public in order to minimize the impact of a public
health emergency.
To assist municipalities in Dakota County to plan, prepare and ready their community to respond to a
public health emergency, including a pandemic influenza outbreak, the Dakota County Public Health
Department and Dakota County Emergency Management are providing an opportunity for financial
assistance. This one -time funding will be awarded to cities that are interested in further developing
their Continuity of Service Plans, and to provide education and outreach to their communities through
collaborations with schools and community -based organizations.
The overall goals of the mini -grant initiative is to:
1 Increase community awareness and preparedness around pandemic influenza and personal
and family preparedness.
2. Develop collaboration among municipalities and community partnering organizations such as
schools and community -based organizations.
3. Increase community readiness by developing and exercising Continuity of Service Plans.
Dakota County will provide technical assistance, informational resources and training to participants
in the mini -grant initiative.
This mini -grant application has been sent to city administrators and police chiefs. Administrators of
community -based organizations and schools will receive an informational letter notifying them about
the mini -grant process and that cities may contact them requesting collaboration.
Funding for this project is provided by the Minnesota Department of Health, and Homeland Security
and Emergency Management (HSEM).
Funding Information:
Each city that submits an acceptable action plan and budget will be eligible to receive $12,000 in
funding Funds must be used by cities to complete internal plans on how they will continue to provide
services in the event of a pandemic influenza, as well as to conduct community outreach and
education on community and individual preparedness.
It is recommended that cities form partnerships with schools and community -based organizations,
including faith -based organizations, in theirjunsdictions to complete the community preparedness
components of the grants. Cities may redirect funding to their community partners to support
outreach and education activities.
Funding Period:
Applications will be accepted until December 22, 2006. Applications will be reviewed to assure that
the activities meet the intent of the grant Applicants will be notified of their application status by
January 12, 2007
Payment of invoices is dependent on receipt of deliverables. If partial payment is required to begin
work, a special request of up to $2000 can be made. All expenses must be submitted by June 15,
2007

Deliverables:
All applicant cities are required to complete all of the City Planning deliverables, and at least
two of the Community Education and Outreach deliverables.
The Community Education and Outreach deliverables may be carried out either by the city or through
collaboration with a community partner, such as a local school or community -based organization.
City Planning deliverables (all are mandatory)
1. Attend an orientation on the mini -grant application.
2. Develop Continuity of Service annex to the Emergency Operations Plan.
3. Develop a Continuity of Service flow chart
4 Participate in one multidisciplinary pandemic related exercise (tabletop, functional, or full
scale) by June 30, 2007 and development of an After Action Report and Corrective Action
Report following the exercise.
Community Education and Outreach deliverables (complete at least two by August 15, 2007)
1. Host a community event or training on pandemic flu personal and family preparedness.
2. Work with existing groups to distribute materials on personal and family preparedness and
pandemic flu (Examples: CERT, Neighborhood Watch programs, faith -based organizations,
Adopt a Highway, youth or seniors group)
3 Work with Community -based organizations in your municipality to develop Continuity of
Service Plans that address how they will assist in meeting surge requests for basic needs
(e.g food, clothes, shelter) in the face of shortages.
4. Develop and implement a plan to communicate critical information (e.g., activation of
emergency operation plans, infection control, or changes in absentee policies) to employees
during a public health emergency including pandemic influenza.
5. Collaborate with organizations in your community to jointly develop a community pandemic
influenza response plan. This plan should articulate how you will coordinate and share
resources, materials, and information to meet the needs of your community during pandemic
influenza.

Salary costs for hiring or reassigning additional staff
Contractor fees
Development or purchase of educational materials
Development or purchase of promotional materials
Costs associated with training to include staff time, space rental, refreshments
Costs of shipping, postage or copying
Other expenses may be allowed please specify in proposed budget
