This item requests the fourth and final payment to Northwest Asphalt, Inc., in the amount of
$21,745.50 for street and utility improvements for Oakridge Estates, City Project #310.
The original contract amount was $214,445.00. Total construction cost, including this final
payment is $169,831.45. The final payment is 21 % under the contract amount.
Upon final payment, a two -year warranty period begins (until September 19, 2002) on the
streets and utilities. The City also accepts these facilities for perpetual maintenance.
Attached is a copy of the application for payment for your information. Staff and SEH, Inc.
recommends approval of this fourth and final payment request and acceptance of the streets
and utilities for perpetual maintenance.
RECOMMENDED ACTION: MOTION TO APPROVE PAYMENT #4 AND FINAL TO
NORTHWEST ASPHALT, INC., IN THE AMOUNT OF $21,745.50 FOR OAKRIDGE ESTATES
STREET AND UTILITY IMPROVEMENTS, CITY PROJECT #310 AND TO ACCEPT THE STREET
AND UTILITIES FOR PERPETUAL MAINTENANCE.

Attached is the fourth and final Application for Payment for the above- referenced project.
The amount due on this application is $21,745.50. This will bring the total project cost to
$169,831.45 which is about 80 percent of the low bid of $214,445.00.
The City has conducted a final inspection and all punch list items have been completed.
Final lien waivers, IC -134 forms and a Consent of Surety are attached for your files.
We recommend the City make payment on this final Application for Payment in the amount
of $21,745.50 to Northwest Asphalt, Inc.
