On October 29, the city received sealed proposals for dental, medical and
long term disability benefits. Attached for your review is a summary and
analysis prepared by the city's consultant, Jeff Azen, of James Bissonnet &
Associates. Mr. Azen has included in the attached booklet an executive
summary on pages i and ii, which provides you a brief but good outline of
the responses received and the recommendations of the Employee Benefit
Committee and of Mr. Azen. Mr. Azen will be at the Council meeting to
answer any questions.
Also attached for your review is a memorandum which details the monthly and
yearly costs for the city and employees by going with the recommendations
of the Employee Benefits Committee. The recommendations are as follows:
(1) Contract with Blue Cross Blue Shield but only have the Aware Gold
Limited with Copay Plan and eliminate the 1st Dollar Plan;
(2) Contract with Canada Life rather than Delta Dental for employee dental
insurance;
(3) Contract with Fortis Benefits for long term disability insurance with
employees paying 100% of the cost and participating on a voluntary
basis.
In my memorandum I point out that if the City only offered the Aware Gold
Limited plan in 1994, it would cost the City an additional $4520 a year
than if the city offered the 1st Dollar Plan and the Aware Gold Plan. This
difference in cost is because in the past the least expensive single
coverage was offered to employees and the 65% family coverage cost was
based on the most expensive family plan. I would like to note that in
comparison to 1993 cost, there is only an $8 total increase per year for
all employees. Therefore, by dropping the 1st Dollar Plan, there is
virtually no increase in cost to the city from 1993 to 1994. Additionally,
by going with Canada Life for dental insurance, the city will save
approximately $4983 a year.
RECOMMENDED ACTION: MOTION TO AWARD THE MEDICAL INSURANCE BID TO BLUE
CROSS BLUE SHIELD, AWARE GOLD LIMITED PLAN WITH COPAYS, AWARD THE
DENTAL INSURANCE BID TO CANADA LIFE, PLAN #2, AND AWARD THE LONG TERM
DISABILITY INSURANCE BID TO FORTIS BENEFITS BASED UPON AN EMPLOYEE
PAID VOLUNTARY PARTICIPATION PROGRAM.

This memorandum will address the recommendations to the City Council for
employee medical, dental and long term disability insurance and the costs
to the city and employees.
MEDICAL INSURANCE:
The recommendation is to award the bid to Blue Cross Blue Shield but only
have the Aware Gold Limited Plan with Copays and drop the 1st Dollar Plan.
The city's benefit consultant, Mr. Azen, feels the city will get better
renewal rates if we only have one plan. I have listed below a cost
comparison which illustrates the additional cost to the city if we only
have the Aware Gold Limited plan.
(1) If city retained both medical plans, the cost breakdown
is as follows:

(3) Based on our current enrollment of 27 single coverage plans and 27
family coverage plans, the city would have an additional expense of
$376.65 a month or $4520 a year if the city only offers the Aware Gold
Plan.
Employees would not pay any additional cost for the Aware Gold single
coverage plan.
Employees would pay $3.62 less a month for Aware Gold family coverage.
DENTAL INSURANCE:
The recommendation is to award the bid to Canada Life and no longer
contract with Delta Dental. There is a reduction in cost to the city for
employee coverage and to -the employee for family coverage as follows:
(1) Based on enrollment of 54 employees in the dental plan for single
coverage the costs are as follows:
Delta Dental: $22.70 x 54 employees = $1225.80
Canada Life: $15.01 x 54 employees = $ 810.54
Total monthly savings to city: $ 415.26
($4983 a year)
(2) Delta Dental Employee Cost for family coverage: $35.80
Canada Life Employee Cost for family coverage: $33.74
LONG TERM DISABILITY INSURANCE:
This will be a new plan for employees but will be paid by the employee.
There will be no cost to the employee except for staff time to administer
the plan.

- EXECUTIVE SUMMARY -
' This Executive Summary and the attached "Request for 1994 Proposal Response Summary and
Analysis" was prepared by the City's Benefit Consultant Jeffrey A. Azen of James Bissonett &
Associates, Inc. The Executive Summary is based on the meeting held Wednesday, November
10, 1993 with the Employee Insurance Committee.
1 BACKGROUND INFORMATION:
' The City currently offers eligible employees a choice between two Blue Cross/Blue Shield
(BC/BS) Plans; Base Plus Major Medical and Aware Gold Limited with Copays. Dental Benefits
are insured through Delta Dental of Minnesota. The City currently does not offer Long Term

Disability Benefits to its employees.
REQUEST FOR PROPOSAL (RFP) RESPONSES:
RFD's were sent to forty-six (46) Insurance Companies and/or Health Maintenance Organizations.
Based on the bid specifications, multiple plan design proposals for Medical, Dental, and Long
Term Disability Benefits, were received from twelve (12) Insurance Companies and/or HMO's.
The RFP Response Summary and Analysis compares proposal responses on the basis of benefit
design, total plan costs, employer and employee contributions, retention costs, medical inflation
trend projections and renewal rate guarantees. The Insurance Committee also reviewed the
results of the Employee Provider Survey.
I. The most competitive proposals for Medical Benefits were received from Blue Cross/Blue
Shield (Preferred Gold Plan), Prudential, Medica and HealthPartners (Group Health &
' MedCenters). These Medical Proposals provided premium savings by reducing access to
Providers. Projected annual medical premium savings ranged from approximately $3,000
(-2%) to 25,000 (-15%).
II. Dental Proposals were received from three (3) Insurance Companies. Projected annual
dental premium savings ranged from approximately $1,500 (-7%) to $3,500 (-16%). The
' highest levels of savings were available only by reducing access to Dental Providers through
Dental Maintenance Organizations and/or Preferred Provider Organizations. However,
' lower levels of savings were available just by changing Carriers.
III. Proposals for a Employee Paid (Voluntary) Long Term Disability Plan were received from
six (6) Insurance Companies. In order to have this Plan, a minimum of 75% of eligible
employees will have to participate.

IBENEFIT COMMITTEE RECOMMENDATIONS:
tL Medical Benefits:
' Since BC/BS did not increase rates for 1994, and since premium savings can only be
generated by limiting access to Providers, the Benefit Committee recommends that the City
Council award the Medical Bid to BC/BS. However, the Benefit Committee recommends
' that only the Aware Gold Limited with Copays Plan be maintained. This recommendation is
based on the recommendation of Jeffrey A. Azen, the City's Benefit Consultant. Having only
one plan will make it easier for the City to get fairer renewals by eliminating the "rate
' gaming" that has occurred in the past between the two plans. One plan will also be easier to
administrate.
t R. Dental Benefits:

' Change Dental Carriers to Canada Life with enhanced new benefits for child(ren)
orthodontics. Even with the enhanced benefits, premiums will be reduced approximately
' 11%. To retain the current plan without Orthodontic Benefits with Delta Dental would have
resulted in an increase in premiums of almost 14%.
IIII. Long Term Disability Benefits:
' Award the Bid to Fortis Benefits conditional to satisfactory enrollment (75% minimum
participation).

' GENERAL INFORMATION
On October 07, 1993, the City of Rosemount provided formal (published) notice of its
' Request for Proposals (RFP's) for Medical/HMO, Dental, and LTD Benefits. The City
required that sealed proposals be received no later than 3:00 p.m., October 29, 1993.
On behalf of the City, James Bissonett and Associates, Inc., the City's consultant and
' agent -of -record, mailed RFP's to forty-six (46) Insurance Companies and/or Health
Maintenance Organizations. RFP responses were received from twelve (12) Insurance
Companies and/or HMO's. All twelve (12) RFP proposals were received within the
' required period.
Jeffrey A. Azen of James Bissonett & Associates, Inc., has condensed the RFP responses
and prepared the following Cost and Benefit Analysis. The City has retained for its
' inspection and review the original RFP Response Proposals.

Benefit Reductions: Fully met specifications.
Benefit Enhancements: There is a "Hold Harness
Agreement" with Network Providers as it relates to charges
(Service Fees) which exceed Delta's Contract Fee Agreements.
Benefit Reductions: Plan Option # 1 fully met
specifications. Alternate PPO Plans A & B have reduced
benefits for services from Non -Network.
Benefit Enhancements: PPO Plan B has In Network
increased benefits for basic services (90%) and major services
(60%).
Benefit Reductions: Fee -for -Service Plan, so employees
are responsible for filing of claims and ineligible coverage to the
extent dentists fees exceed the 90th percentile of charges in the
zipcode areas applicable. Fluoride treatment for dependents
under age 14 (vs. age 19).
Benefit Enhancements: Bitewing x-rays every six months
(vs. twelve months); fluoride treatment every six months (vs.
twelve); space maintainers covered at 100%, no deductible (vs.
80% subject to deductible); sealants covered at 80%, subject to
deductible (vs. no coverage).
Benefit Reductions: Fully met specifications
Benefit Enhancements: Dental Maintenance Organization
(DMO) offers no deductible (there is a $5 office visit copay)
and increased benefits for most services (vs. current plan).
