In January a recommendation came before the City Counc 1 rega�ing a
"Community Comment Card. ° The idea was proposed by Mr. Don Sinnwell . Mr.
Sinnwell was supported in this idea by members of the Chamber of Commerce.
The example of the type of card used was from the City of Burnsville.
Based on comments received at the meeting it was suggested that this may be
a good format to use to get public comment to the City Council on concerns,
complaints and compliments on the services being provided, or lacking, by
the City.
I have asked my staff to do additional research on this matter to see how
other cities have utilized this approach to gather information. A brief
report in that regard is attached.
_. As you can= see by the information provided, citi�s go. about this process in
different ways. What type of information requested or considered is
generally related to "services provided" . To go beyond this it would seem
more appropriate to consider a full fledged citizen survey. o
Based on that review and the interest of the Chamber and others who have
brought this to you for consideration I would suggest the following:
1) Establish an ad-hoc committee of seven (7) people with representation
from the Chamber (2) , the City Council (1) , and the City Commissions
�4) .
2) This committee would be charged with developing the format for a
Rosemount Comment Card_
3) The process for the distribution of the card, and the collection of
information also be recommended by this committee. It would seem that
the distribution can be handled fairly effectively and at a minimal
cost.
4) Suggested cost and appro�riate funding should be suggested depending�'
on what type of information is sought. I would suggest that cost
sharing from sources other than City funds be considered.
5) This committee would report back to the City Council with a
recommendation at the first meeting in April .
RECONIl+SENDED ACTIONe Motion to approve the forming of a Citizens Comment
Card Committee to draft recommendations on the format use and payment
of cost for the use of such a card and this committee report back to
the City Council on April 6, 1993 .

I have contacted the following cities by phone to research the
use of residents written input and surveys.
These cities have no routine surve�r or comment card:
Bloomington
Fridley
Apple Valley
Eagan
Farmington
These cities did routine surveys and some used comment cards:
1. Richfield - inserts 50 surveys randomly into water billings
each month for 14 months;- they had a poor response and are
- discontinuing. No eomment cards. �
2 . Chanhassen = only Parks & Rec does survey on programs. No
_ comment card._
3. Lakeville - No comment card, however they conduct a phone
survey by Decision Resources every two years. The cost is
budgeted for, -�2, 000 . This is their "report card" .
Adm. Bob Erickson had experience in Montana using comment
cards. They were in triplicate forms, required 48 hour
response time by department head. 98% were positive
comments, praise was given to indivic�ual employee.
4 . Burnsville - Yes, they use comment cards . Cards are placed
- - at each counter in City Hall plus any other city facilities
ie. ice rinks, galf course, community center. They also do
randam mailings to approximately every 4th fire
call/rescue/police call. They have a poor or low r�turn on
cards issued this way, all are positive that have been
returned at this point.
The process established is that after the card is received,
the department involved is determined, the department head
is notified, then action is taken. The information is kept
in a data base and a summary of the information from the
Comment Card is furnished to the City Council .
A citizens committee was formed to design the card. Once '
this task was accomplished, the committee quit meeting.

5 . Eden Prairie - "How Are We Doing?0 Card used. It is kept on
all counters and handled through each department.
6 . Woodbury - 6 times a year, the "Feedback" card is the center
fold of their city magazine. The "Feedback° card is also on
all counters in City Hall.
7. Plymouth - ��Talk To Us° Card used at all counters. Three
times a year this card is handed to everyone who enters City
Hall for one week.
