Specialist DATE: May 31, 1989 SUBJ: Rosemount "Meals on Wheels" Pursuant to directiott given at the last City Council meeting, staff has worked with the proposed Advisory Board of the Rosemount home delivered meals program to develop a policy statemen[ and agreements to be entered into by the City of Rosemount (as program sponsor), the Program Advisory Board, and Apple Valley Health Care Center (meal suppliers). The Policy Statement for Rosemount Meals on Wheels explains that the grogram will formally be called "Rosemount Meals on Wheels" and will serve the cities of Rosemount,'Coates, and that area of Valley Park (Apple Valley) which has a Rosemount mailing address. In addition, various issues which will be facing the Advisory Board are addressed. The agreement between the City af Rosemount and the Rosemount Meats on Wheels Advisory Board outlines the rights and responsibilities of each. The main obligations of the City of Rosemount as the Program Sponsor include (1) taking ultimate legal and financial responsibili[y for the program; (2) providing the program with tax exempt status; (3) providing an account to be used by and for the program; and (4) groviding insurance coverage for the program under the City's current insurance policy. The Advisory Board will be responsible for all of the operational aspects of the program. The agreement between the Rosernount Meals on Wheels Advisory Board and Apple Valley Health Care Center is constructed in the same format as the previous agreement. The basic rights and responsibilities of each are outlined. Staff respectfully requests City Council approval of (1) Ehe "Policy Statement for Rosemouttt Meals on Wheels'; (2) the "Agreement Between Apple Valley Health Care Center and The Rosemount Meals on Wheels Advisory Board"; and (3) the "Agreement Between The City of Rosemount and The Rosemount Meals on Wheels Advisory Board." Upon appraval, staff recommends that the City of Rosemount enter into said agreement with the Advisory Board. + • � y � � POLICY STATEMENT FUR ROSEMOUNT MEAL5 ON WHEELS I. Officiat Program Name: ROSEMOUNT MEALS ON WHEELS I�. Program Sponsor: City of Rosemount III. Pragram Boundaries: The cities of Rosemount, Coates, and that area of Valley Park (Apple Valley) which has a Rosemount mailing address. IV. Advisory Board (see: Agreement between Sponsor & Advisory Board) A. The Advisory Board will be made up of persons representing the different sectors and civic organizations of the Rosemount community. At all times the Advisory Board will have at least one member representing the Program Sponsor. B. Officers for the Advisory Board wi(1 consist of one Chairperson, two Vice Chairpersons and one Secretary. These officers will be electeci yearly in the month of July by the other members of the Advisory Board. C. The Advisory $oard members wil! be required to commit to a term of at least one year. If an Advisory Board member does not repeat a Eerm on the Board, he/she will be responsible for finding a replacement from that same sector of the community which he/she represents. D. The Advisory Board will have regular meetings quarterly. Special meetings may be called at any time by the officers of the Advisory Board or by tbe Program Sponsor. E. Special needs of the Rasemoun[ Meals on Wheels Program will be served through Che formation of comrnittees including: - FinancelFundraising Committee - Human Retations Committee (personnel) - Public Relations Committee V. Client Issues A. Eligibility guidelines: Program recipients must be hornebound elderly and/or disabled persons within the Program distribution area. B. Subsidy Policy: Persons admitted to the Program will be given a subsidy for the payment of their meals only through the referral of a social service professional or an in-home assessment by the Program Coordinator. Subsidies will be provided through Program donations. 1 • _ ^ � • C. All persons receiving subsidies lhrough lhe Rosemoun[ Meals an Wheels Program will be subject to a periodic reassessmenx of need by the Program Goordinator. D. All persons wishing to participate as meal recipients in the Rosemount Meals on Wheels Program will be required to properly coraplete applications for usc by the Progrum C�c�rdinalor and Apple Valley Health Care C�nter. VI. Meals (see: Agreemenl between Sponsor & AVHCC) A. Meals and supplies will be purchased from Apple Valiey Health Care Center and distribu[ed at cost. B. Therapeutic diets shall be served as required by the clients' health care professional. Client application forms will assist in determining this informa[ion. C. Notice of cancellation of a meal(s} must be given to the Program Coordinator the day before the meal is not wanted (24 hour notice). D. All meals will be delivered within one hour of the .time they are packed at Apple Valley Health Care Center. E. Meals will be distributed Monday through Friday, except major holidays: New Year's Day; Memorial Day; July 4th; Labor Day; Thanksgiving; and Christmas. VII. Personnel A. Rosemount Meals dn Wheels does not assume any responsibility for liability insurance for paid or t�olunteer personneL B. The Hurnan Relations Committee has dctermined that the Program Coordinator will be paid $200 per month. Substitute coordinators will be paid $10 per day. C. The Advisory Board is responsible for recruiting and recognizing all Program Volunteers. The Program Coordinator will be responsible for orientation and scheduling of said volunteers. Vlil. �iaunclal Issues A. The Finance/Fundraising Committee will be responsibte for solicicing donatiotts from those sectors of the community which they represent as well as other area organizations. 1 + • » � � B. An annual budget will be prepared by the Finance/Fundraising Committee with assistance from lhe Advisory Baard. Upon the Advisory Board's approval, the annual budget witl be submitted to the Sponsor for review and approvaL C. Payments and donations may be sent to the Rosemount City Hall. The Program Coordinator will be responsible for the monthly billing of clients and requesting funds from the Program Sponsor for payment of expenses. D. All Program records will be kept by the Program Coordinator and will be available for review at any time by the officers of the Advisory Board and [he Program Sponsor. �, , � -� � � AGREEMENT ROSEMOUNT MEALS DN WHEELS ADVISORY BOARD AND APPLE VALLEY HEALTH C�hRE CENTER This agreement is made this day of , 1989, by and between the ROSEMOUNT MEALS ON WHEELS ADVISORY BOARD, hereinafter referred to as "The Board," and APPLE VALLEY HEALTH CARE CENTER, 14650 Garrett Avenue, Apple Valley, Minnesota, as the Rosemount Meals on Wheels meal provider, hereinafter referred to as "AVHCC". WHEREAS, all actians of The Board are subject to the approval of the City of Rosemount as the Rosemnunt Meals on Wheels Progr$m Sponsor; WHEREAS, the City of Rosemount as the Rosemount Meals on Wheels Program Sponsor has ultimate reponsibility for the Program; NOW, THEREFORE, in consideration of the provision herein cantained, the parties agree to the following: 1. AVHCC will prepare the noon meals regularly served to their residents for the Rosemount Meals on Wheels Program every weekday (Monday through Friday} except major holidays. 2. AVHCC will provide diabetic, low salt and low fat/cholesterol meals to accomodate special dietary needs. 3. AVHCC will supply all meal containers no[ pravided by the Rosemount McDonalds. 4. AVHCC kitchen staff will paek the hot and cold foad into separate containers and have them ready by 11:45 a.m. 5. AVHCC will provide a telephone and an area for the Program Coordinator to use during the hours of 11:00 a.m. through 1:00 p.m., Monday through Friday. 6. AVHCC will provide the use of their telephone aumber (431-7700) to be given out as the Rosemount Meals on Wheels number, with the request that calls be made between 11:00 a.m. and 1:00 p.m. 7. The Board will pay AVHCC $3.00 for each meal prepared for the program. Payrnents for said meals will be made on a monthly basis. 8. The Board will transport any meal packing materials donated by Rosemount McDonalds to the AVHCC kitchen. 9. The Board will give the center a [hirty (30) day notice of discontinuation of service. ' . Notary Public �. � � ! � +� � � , AGREEMENT THE CITY OF ROSEMOUNT AND THE ROSEMOUNT MEALS QN WHEELS ADVISORY BOARD This agreement is made this day of , 1989, hy and between THE CITY OF ROSEMOUNT, a body public and politic, hereinafter referred to as the "City", and THE ROSEMOUNT MEALS ON WHEELS ADVISORY BOARD, hereinafter referred to as the "Board", for Rosemount Meals on Wheels, a nonprofit program, hereinafter referred to as the "Program". W1iEREAS, the City is �roviding [he Pr�gram with tax exempt status; WNEREAS, the City is providing an accouni to be used by and for the Program; WHEREAS, the City is providing insurance coverage for the Program under the City's own insurance policy; NOW, THEREFORE, in consideration of the provisions herein eontained, the parties agree to the following: 1. The City will have uitimate legal and financial responsibility for the Program. 2. The City will have the right to terminate the Program and distribute any assets if the City determines that the Program is financially insolvent or has insufficient community support to be properly administered. 3. The City has the right to turn the Program over to another sponsoring agency and/or to ailow the Board the option to incorporate the Program as an independent corporation. 4. The City will provide postage and copying costs for the Progra�n. 5. The City will have the right ta approve or deny the annual Program Budget prepared by the Program $oard. 6. The City may recommend candidates for the coordinator position as well as approving Baard recommendations regarding hiring, job description and salary of the coordiaator. 7. The City will review program progress evaluations submitted by the Program Board. 8. The Board will raise funds for the Program and deposit them in the checking account set up by the city. 9. The Board will propose an annual Program Budget fo the City. ld. The Board will interview candidates for the Program coordinator position and will make recommendations to the City regarding hiring, job descriptiou and salary. � _ . � 11. The Board will assist the Program coordinator with decisions on the day to day operations as well as the overall administration af the Program. 12. The Board will publicize the Program to both clients and volunteers. It will recruit and recognize Che Program volunteers. 13. The Board will communicate the needs and resources of the Program to the various community groups it represents. 14. The Board will evaluate the progress of the Program and presEnt these reports to the Ci[y for their review.
