RECOMMENDED ACTION: Motion to approve Change Order #4 for the Family Resource
Center Project.
ISSUE
In the summer of 2019, the City Council awarded a bid for the expansion of the Family Resource Center.
The expansion plans will add 3,320 square feet to the facility. During the construction of the project a few
changes have been needed that have resulted in staff requesting the City Council to consider approving
change order #4.
The changes that are incorporated in the attached Change Order #4 are the following:
CR 20 Ceiling projector receptacle
CR 21 Drywall CREDIT
CR 22 Tree removal
CR 23 Translucent Ceiling Tiles
CR 24 Mail Box and Pedestal
CR 25 Hallway Chair Rails
CR 26 Barn Door Hardware
CR 27 Separate Ground Neutrals
CR 29 Lockable Data Cabinet
CR 30 Occupant Load Signage
CR 31 Key Lock Box
CR 33 Additional Data Cable
The total of the twelve requests is $9,904.18. Please see the attached AIA Document and architect letter
for further details.
RECOMMENDATION
Staff is recommending that the City Council approve Change Order #4 for the Family Resource Center
expansion project.

SUBJECT: Change Order #04 Review
The following is a summary of the items being submitted as Change Order #04 for the Rosemount
Family Resource Center project.
Background:
The construction on the Rosemount Family Resource Center project has reached substantial
competition. There are just a few items left to finish on the exterior of the building in the spring and
the tenant, 360 Communities has moved into the space
During the past few months several items have come up to improve the facility. These items are
summarized below.
Description of Changes:
The changes that are incorporated in the attached Change Order #04 are the following:
A. Electrical Changes: These changes addressed adjustments to equipment and wiring to best
meet code and the needs of 360 Communities.
B. Drywall Credit: The specifications called out an upgraded high-quality drywall compound,
but a lower grade compound was used for part of the project, rather than redoing the drywall,
a credit was provided.
C. Site Changes: These changes included a new mailbox, removal of a tree and a lockbox.
D. Miscellaneous Interior Changes: This change involved translucent ceiling tiles to introduce
more daylighting into the Activity Room, revised door hardware, additional chair rails and an
Occupant load sign.

Pricing:
The following are the pricing for each group of Proposal Request items listed above and included in
this Change Order. All prices have been reviewed by CNH Architects and our consultant team and
have been determined to be within appropriate unit costs for the work proposed.
A. Electrical Changes Add $5920.20
B. Drywall Credit Add ($700.00)
C. Site Changes Add $1580.28
D. Miscellaneous Interior Changes Add $3103.70
Total Add $9,904.18
New Contract Total $1,030,381.85
Original Contingency (10%) $98,280.00
Contingency Balance $50,698.15
Conclusion:
The items in the Change Order represent recommended upgrades to best serve the city and staff
using this facility. As noted above CNH Architects, our team, and staff have reviewed the pricing
shown above and are comfortable as being appropriate for the work involved. CNH Architects
recommends approval of this Change Order as in the best interest of staff operations and long-term
City value.
