ATTACHMENTS: Resolution, Payment Voucher, Bolton & Menk Recommendation Letter, Map APPROVED BY: LJM RECOMMENDED ACTION: Motion to Approve Payment 6 and Final to EJM Pipe Services, Inc. for Greystone 7th Street and Utility Improvements, City Project 2018-10 and Accepting the Improvements for Perpetual Maintenance BACKGROUND This item is a request for Payment 6 and Final to EJM Pipe Services Inc. for Greystone 7th Street and Utility Improvements, City Project 2018-10. The project is funded by the developer. The final construction amount of $932,652.81 is 0.6% less than the sum of the contract amount plus field/change orders: $913,868.95 + $24,619.47 = $938,488.42. The change orders include work to replace curb and gutter damaged by snow plowing and watermain work including an extension to loop a long cul-de-sac dead end. RECOMMENDATION The City has received documentation from the contractor required for final payment, including 2-year maintenance bond, state withholding tax forms, lien waivers and consent of surety to final payment. Staff recommends that Council approve Payment 6 and Final for the project and approve the resolution to accept the improvements for perpetual maintenance. CITY OF ROSEMOUNT DAKOTA COUNTY, MINNESOTA RESOLUTION 2020 – 105 A RESOLUTION ACCEPTING PUBLIC IMPROVEMENT FOR GREYSTONE 7TH ADDITION CITY PROJECT 2018-10 WHEREAS, the contractor for Greystone 7th Addition has submitted documentation for City acceptance of the public improvements; WHEREAS, the City received a letter from Bolton & Menk, Inc. confirming the public improvements were installed in accordance with the approved plans and specifications and finds the improvements acceptable; BE IT RESOLVED, by the City Council of the City of Rosemount, Minnesota. The work completed under said agreement is hereby accepted for perpetual maintenance and ownership. ADOPTED this 5th day of October, 2020. William H. We find the City street and utility work has been satisfactorily completed by the contractor, EJM Pipe Services, Inc., in accordance with City standards. For your information, we have tabulated the following utility tests and inspections, and dates performed: Inspection of water and sewer (Final) – June 2019 Television inspection of sanitary sewer – October 2018 Water main pressure test – October 2018 Water main conductivity test – October 2018 Water samples for bacteriological test – October 2018 Water main turned on for service – October 2018 The street construction was performed by McNamara Contracting Inc. for EJM Pipe Services, Inc., The street work for this project consisted of concrete curb & gutter, aggregate base, concrete sidewalk, and bituminous paving. For your information, we have tabulated the following inspections and dates of work performed: Inspection of bituminous base, concrete curb, and concrete sidewalk (Final) – June 2019, May 2020 Pour Concrete Curb – November 2018 Pave Bituminous Base Course – November 2018 Pave Bituminous Wear Course – June 2020 Enclosed are the IC134 Forms and Lien Waivers submitted by the contractor. Also included is the two- year Maintenance Bond as required by the Subdivision Agreement. We are recommending to the City of Rosemount that this utility and street improvement project be presented for final approval and acceptance as described in the Subdivision Agreement.
